Icon woes

I think I'm going barmy here. I have an icon file, with the appropriate number of icons in all sizes and colour depths.
It is included in my resource file - the only icon file.
But when I compile, and run the program, all the icons are OK in the program, except for the one displayed when in Explorer.
And THAT one is not included in the icon file at all, and is nowhere on my hard disk.
It is completely alien, and I can't get rid of it. Where is it coming from?

I know windows will take an icon and resize it. Does it look at all like any of your icons?

No, it's not the same icon at all. Not at all like the icons in the icon set
It's not even the default Windows application icon

Edit: Drastic measures. Deleted the icon cache files and re-booted - all OK now!
